The 2 a.m. brake line burst on I-80 that made me question my own flaring job

I replaced that rear line last Tuesday in Des Moines, double-flared it perfect, but the rusted bracket rubbed through the new line in under 48 hours because I skipped the grommet, so has anyone else had a repair fail from something you didn't even think to check?
