My anvil stand felt off all week, then on Friday the whole thing tipped sideways mid-strike.

I was working on a 3/8 inch scroll bracket and the stand just buckled under the hit from my 4 pound hammer, and now I'm wondering if I should just bolt it to a concrete pad or build a new wooden base with a wider footprint - has anyone else had a stand fail like that?

Bolt it down. Concrete pad with some rubber matting underneath. Never had a tip since I did that.

4 lag bolts through the frame into the concrete and mine hasn't budged an inch in 3 years. Added rubber stall mats from Tractor Supply underneath it too, that combo works way better than I expected.

My 4 pounder took out a 2x6 base last year. Go with a wider wooden base, way easier to fix if it shifts.
