Sewing frame mishap cost me a whole Tuesday afternoon

Last month I was putting together a 400 page journal for a client and my sewing frame slipped off the bench right as I was tensioning the cords. The pages scattered everywhere and I had to re-collate the whole text block from scratch, which took about four hours. What made it worse was I found the frame's base screw had been loose for weeks, I just never checked it. Has anyone else had a setup failure that wiped out a full work session, and what do you do to keep your equipment from betraying you mid-project?
