Saw a buddy's glue-down job fail after 3 months
He used the wrong trowel notch size for commercial carpet tile. Had to rip up 200 square feet in a downtown Denver office. Anyone else see this happen with pressure sensitive adhesive?

1/16" V-notch" is actually pretty standard for some carpet tiles, not always the issue.

Had a buddy do a glue-down job in a brewery taproom last year. Used a 1/16" V-notch for 18" tile and it just peeled right up after two months. The adhesive never got enough transfer through the backing because the notch was too small. Had to pull every tile, scrape the old glue, and re-do it with a 1/8" notch. Took him a whole weekend and he lost the account.
