The broken pallet jack that almost cost me a safety audit

I was unloading a truck at the warehouse last Friday and my pallet jack just locked up halfway through... couldnt move it forward or backward. I had 20 pallets of canned goods sitting on the dock and the delivery driver was getting antsy. I ended up grabbing a crowbar from the maintenance room and prying the brake release pin back into place, which took maybe 10 minutes. The weird part is I found out later that same pin had been reported broken two weeks ago and nobody fixed it. Has anyone else dealt with a piece of equipment that management just ignores until it actually fails?

That same pin being reported two weeks ago and nobody fixing it... classic. Grab a sharpie and write the work order number directly on the handle next time. Makes it real hard to ignore when they have to look at it every time they walk by.
