My go-to toner dried out a client's scalp, had to redo the whole color

Used the same toner I've trusted for 2 years on a regular last Friday, and her scalp went red and flaky within an hour. The brand must have changed something in the formula without saying, because I've done this exact mix on her 6 times before with zero issues. I had to strip it off, apply a calming treatment, and rebook her for a gloss next week for free to make it right. Client was cool about it, but it made me wonder if I should be patch testing every repeat client, not just the new ones. Anyone else get burned by a 'trusted' product changing on them out of nowhere?
